Portsmouth, VA — A female worker at APM Terminals was killed on Monday, March 28, 2011, in a fork lift accident. The fatal workplace accident occurred just after 8 a.m., at the APM Terminals in Portsmouth, Virginia, reported the Daily Press.
According to police officials, 38-year-old Paula Bellamy of Spingbloom Drive in Portsmouth was working as a “slinger,” or also known as a guide to a crane operator, for Ceres Marine Terminals Inc. when she was fatally hit.
Bellamy was reportedly standing on a pier, using a radio to guide a crane operator, when the forklift operator—whose vision was obstructed by the containers he was moving—struck the victim, according to a port authority spokesman.
Responding emergency crews rushed Bellamy to Sentra Norfolk General Hospital where she was pronounced dead at 8:59 a.m.
Police and the U.S. Occupational Safety and Health Administration (OSHA) are investigating.
