Indianapolis, IN — A warehouse floor collapsed at an Indianapolis salad-dressing maker, killing a 50-year-old fork lift operator. The fatal workplace accident occurred at around 6 a.m. at Royal Food Products, 2322 E. Minnesota St., as reported by the Indianapolis Star.

According to officials, Alan Beaty, 50, of Clayton was moving bottles of vegetable oil into boxes with a fork lift when the floor collapsed from underneath him, causing the fork lift to fall 14 feet into the basement. Large barrels of oil and other debris then fell onto the forklift from the first floor.

The owner of the Southeastside business said parts of the first floor had been reinforced for the use of the fork lift, but apparently Beaty had driven over a part of the floor that was not reinforced.

Responding emergency crews were able to remove Beaty from the wreckage at 3:20 p.m. He did not survive the accident.

Code-enforcement officers reported to the scene to inspect the building’s structural integrity.

The Indiana Occupational Health and Safety Administration and Marion County Coroner’s Office authorities also arrived at the scene to conduct investigations.
