Milwaukee, WI — An industrial accident on the south side killed a 22-year-old worker on Tuesday, June 5, 2011. The fatal industrial accident occurred when a 6,000-pound steel bar fell on the worker at Acme Galvanizing Inc., 2730 S. 19th St., at around 4 a.m., reported JSOnline.com.
According to police officials, a steel bar had not been properly secured while attached to an overhead crane, when it fell from 10 to 15 feet in the air. Benjamin H. Alcorn, a crane operator at Acme Galvanizing, was subsequently crushed under the bar.
According to the report, Alcorn wasn’t standing in a designated area that operators are supposed to stand in.
The Milwaukee Police Department, the Milwaukee County district attorney and the Occupational Safety and Health Administration (OSHA) are investigating the industrial accident.
Acme Galvanizing has inspected the factory three times in the past 10 years, according to OSHA records. Acme was cited for four violations as a result of a July 2 complaint. In 2005, Acme was issued citations for 21 violations and $15,875 in fines, which were later lowered to 10 violations and $2,625 in fines.
