Milwaukee, WI — A Superior man was killed Saturday, November 12, 2011, when his arm was severed after it got caught in a moving conveyor belt on a ship docked in Milwaukee. The fatal workplace accident occurred around 9:15 a.m. aboard the SS St. Mary’s Challenger, which was docked near Kinnickinnic Avenue, reported JSOnline.

According to reports, Ronald L. Hackensmith was working alone by the conveyor belt three decks below the main deck when his arm became caught in the conveyor belt, causing it to be severed at the shoulder.

Ship logs revealed that there was a hydraulic oil leak when Hackensmith asked other crew members to shut down the engine and turn it back on. The engines were shut down and restarted, before Hackensmith requested that they be shut down again. According to the log, Hackensmith never asked for another start-up but the engines were restarted, and were running when Hackensmith became caught in the conveyor belt, JSOnline reported.

Accident investigators have listed the fatality as an “accident.”

Milwaukee police are investigating the fatal ship accident.
